• Community Partners for Children (CRCG-CPC) is a community partnership that meets with families to create individualized service recommendations to support the child and family.

• Children and Youth of Travis County
• Children and Youth between the ages of 0-22 years of age
• Children and Youth with multiple system involvement
• Children and Youth whose Physical Challenges, Mental Health Challenges and/or Developmental Disabilities significantly impact their ability to function in home, school and/or the community

• Create individualized service recommendations to support the child and family

Community Partners for Children (CRCG-CPC) is the eligibility meeting to facilitate enrollment and/or access to these programs:

• The Children’s Partnership (TCP) – A program designed to provide wraparound supports to families with children who have significant mental health challenges.
• CRCG-CPC Bridge Support – A program designed to support families who are either not eligible for or on the waiting list for wraparound supports
• Waco Center for Youth – CRCG-CPC provides support and prioritization for applications for placement submitted to WCY
• ICF-IID – CRCG-CPC provides support to seek placement through the state Intermediate Care Facilities for Individuals with an Intellectual Disability or Related Conditions program.
• CRCG-CPC Partners are also able to provide access to a variety of local programs, services, supports and evaluations

• Anyone who is concerned about a child with complex needs

• The family has accessed personal resources and
• Eligible community resources and
• Involved Agencies have exhausted internal resources BUT
• The Child/Youth continue to have unmet needs

• Family will receive a call from either a CRCG-CPC representative or the referring agency to:
  • Ask if the family is interested in participating with CRCG-CPC
  • Gather Information about child and family
  • Answer any questions family may have
  • Discuss next steps
• Complete a Referral Packet
  • Packet completed by family and referring agency
  • Family and referring agency gather documents for referral packet
  • Packet will include information on child and family’s history, strengths, challenges, and successes.
• Packet submitted to CRCG-CPC Liaison or CRCG Coordinator if TCJPD is referring agency
• CRCG-CPC session is scheduled

• Parent participates in session
• If desired parent brings support person (s)
• Each family meets with the group for approximately 45 minutes
• Parent and/or agency representative briefly tells about the child and family’s strengths, current challenges, successes, and needs
• At the session the parent and the group will create a list of service recommendations for the child and family
  • CRCG-CPC members may ask questions
  • CRCG-CPC members will share information helpful to the family team
  • Resources will be identified
• The CRCG-CPC recommendations will be developed and written to include tasks, resources and the persons responsible for each task
• A CRCG-CPC partner will be identified to work with the family to implement the list of recommendations
• o After the session, the recommendations will be reviewed by a CRCG-CPC representative with the parent and/or caregiver. A copy of the CRCG-CPC Recommendations will be provided to the family and their service team.

CRCG-CPC is comprised of committed public and private community representatives and families, who bring their expertise and resources to create a continuum of care for children and families of Travis County.

CRCG-CPC currently has approximately 20 members who actively participate in the sessions.

CRCG-CPC meets twice a month on the second Wednesday and fourth Thursday with families and their agency representatives.

The meetings are from 11:00 – 5:00. With the first few hours a review period for the CRCG-CPC members and then four 45-minute sessions with the individual families.
